Please complete this request if you are a F-1 or J-1 student who is degree-seeking, and will graduate from your academic program this semester.
Do not complete this request if any of the following are true:
You have already applied for OPT based upon your current degree.
You are a J-1 Non-Degree seeking student.
You are a Pathway student progressing into your degree program OR You are completing one degree and immediately continuing on at ISU for another degree program. You should instead complete a Change of Level Request.
